My 4-H Career Record Book

We encourage all members to apply for county and state awards and trip opportunities.  We are trying to make it as painless as possible to apply.  Last year we created a form that can be used to apply for county opportunities such as Camp Counselor, Livestock Sale Committee, and Junior Fair Board (instead of the Ohio Achievement Form requirement), and this two-page form made it much easier for our teens to apply.

This year we have revised the Ohio 4-H Achievement application form that is required when applying for county trips and state opportunities.

It can be an intimidating task to complete these forms if you have not kept track of your 4-H career. To help with this process, we have created a booklet called My 4-H Career Record Book.  It includes 12 pages that will allow you to record information about each year of your 4-H career.

You may either download the booklet by clicking on the link below OR you may purchase from the Extension Office (printed on 4-H green paper!) at just $1 each to cover printing costs.

4-H Junior Camp was GREAT!  107 campers, 25 counselors, and 9 adults volunteers accompanied Sheila and I to camp.  We had a good weather, everyone remained healthy, all campers behaved well -- it was a GREAT camp.
